1. Right to Respect and Dignity
You have the right to considerate, respectful care from all Sivo Health providers and staff at all times and under all circumstances, with recognition of your personal dignity. You have the right to be free from discrimination based on race, color, religion, gender, national origin, sexual orientation, or any other legally protected characteristic.
2. Right to Information
You have the right to obtain from your healthcare provider complete and current information concerning your diagnosis, treatment, and prognosis in terms you can reasonably be expected to understand.
You have the right to receive information about the Sivo Health platform, our providers, and our clinical guidelines.
3. Right to Privacy and Confidentiality
You have the right to expect that all communications and records pertaining to your care will be treated as confidential by Sivo Health, except in cases such as suspected abuse or public health hazards when reporting is permitted or required by law.
4. Right to Participate in Decisions
You have the right to participate in decisions involving your healthcare, except when such participation is contraindicated for medical reasons. This includes the right to accept or refuse medical treatment to the extent permitted by law and to be informed of the medical consequences of such refusal.
5. Right to Transparency in Billing
You have the right to receive an itemized and detailed explanation of your total bill for services rendered via the Sivo Health platform, regardless of the source of payment.
6. Right to Voice Grievances
